If you’ve known me for any length of time, you know this topic fires me up — in a good way.

When I first started practicing, estrogen therapy was the standard of care for menopausal women. It wasn’t controversial. It wasn’t scary. It was simply what we used because it worked — and because decades of clinical experience supported its safety.

Then came the Women’s Health Initiative (WHI) in 2002.

And overnight, everything flipped upside down.

Not because estrogen suddenly became dangerous — but because the study was completely misinterpreted, the media ran with a fear-based story, and women all across America suffered the consequences.


What Actually Happened?

Let me break this down clearly:

  • The WHI did not show that estrogen caused breast cancer.

  • The estrogen-only group actually had lower breast cancer rates.

  • No increase in breast-cancer mortality has ever been shown.

  • The average woman in the WHI was older, years past menopause, and using outdated hormone formulations.

  • The results were never reproducible.

  • The Nurses’ Health Study and other large trials found no increased breast-cancer risk.

  • Even today, women with estrogen receptor–negative breast cancer can safely use estrogen.

And here’s a key point most people don’t realize:

In the WHI and other studies, ALL estrogens were lumped together.

Older synthetic estrogens.
Non-bioidentical forms.
Higher, non-physiologic doses.
Even synthetic hormone combinations similar to those used in birth control pills.

All thrown into one bucket.

This is like being lumped in with your trouble-making cousin, and you are the good Catholic boy:
You’re totally different people, but you get judged by their behavior.

Bioidentical estrogen — the form identical to what your body naturally makes — got a bad name simply by association.


Why I Stayed the Course

While the rest of the medical world panicked, I continued prescribing estrogen because the real science never matched the fear.

I’ve always believed in:

  • understanding physiology

  • reading the data myself

  • respecting reproducibility

  • and treating each woman individually

For the right woman, at the right time, in the right form, estrogen is one of the most effective and safest therapies we have.

It supports:

  • hot flashes and night sweats

  • sleep, energy and mood

  • memory and cognition

  • metabolism

  • bone density

  • cardiovascular health

  • vaginal, pelvic, and urinary health

  • libido

Women deserve these benefits — without being scared away by headlines from 20 years ago.


Science Matters — But Only When It’s Understood Correctly

Science is powerful.
But it can also be misused when:

  • studies are oversimplified

  • data is lumped together inappropriately

  • headlines distort the truth

  • or fear becomes a tool to control or manipulate

Science teaches us about populations.
But
you are not a population.

You’re an individual with unique physiology, stressors, genetics, symptoms, and goals.

And this is where science alone falls short.

This is where the art of medicine matters — something no algorithm, no AI tool, and no standardized guideline can replicate.

Unfortunately, insurance companies and government-driven healthcare tend to push patients into categories, flowcharts, and “study groups.”

“Women age 50–60 = do this.”
“Women with X risk factor = don’t do that.”
“Everyone in this group = same treatment.”
