Microneedling for Acne Scars: What Results to Expect and How Many Sessions You'll Need

Acne scars are one of the most common skin concerns I see in our practice—and one of the most emotionally heavy. You've done the hard work of clearing your skin, only to be left with a reminder of what you went through. The texture, the shadows, the way scars catch light in photos—it can feel defeating.

The good news is that microneedling is one of the most effective, science-backed treatments we have for improving acne scarring. But like most good things in aesthetics, it requires patience and realistic expectations.

One of the most common questions I get from patients considering microneedling is not "Does it work?" but rather:

  • What will my skin actually look like after?

  • How many treatments do I need before I see results?

  • And is it going to be worth it?

Let me answer all of that.


Why Microneedling Works on Acne Scars

Acne scars form when inflammation damages the deeper layers of the skin, disrupting collagen production and leaving behind either depressed (atrophic) or raised (hypertrophic) texture. Microneedling addresses this at the source.

We perform microneedling at Columbia Shores Regenerative Health using the SkinPen Precision—the only FDA-approved microneedling device on the market. It works by creating thousands of controlled micro-injuries in the dermis, which triggers your body's natural healing response. New collagen forms. Fresh skin cells are generated. Over time, that process begins to fill in and smooth the uneven texture left behind by acne.

What makes microneedling particularly well-suited for acne scars is that it works with your body—not against it. There's no foreign substance, no significant disruption to surrounding tissue, and no long recovery. Your skin is doing the work; we're simply giving it the signal to start.


What Results to Expect—and When

This is where I want to be upfront with you: microneedling results are gradual. You will not walk out of your first appointment with visibly clearer skin. What you will notice is that your skin looks flushed, feels slightly sensitive, and may have some minor pinpoint redness for 24–48 hours. That's completely normal.

Here's a more honest timeline:

After Session 1

  • Skin may feel tighter and slightly smoother as initial healing begins

  • Most patients don't see dramatic visible change yet

  • Your skin is beginning the collagen-building process beneath the surface

4–6 Weeks Later (Session 2)

  • Subtle improvements in texture start to become noticeable

  • Pores may appear smaller

  • Skin tone often looks more even

After Session 3 (and Beyond)

  • The most significant visible improvement typically appears

  • Acne scars—particularly rolling and boxcar types—begin to soften and fill in

  • Skin surface looks smoother, more refined, and healthier overall

  • Full collagen remodeling continues for up to 3–6 months after your final session

The results keep improving after you're done with your sessions, which is one of the things I love most about this treatment. You're not just addressing the surface—you're rebuilding the structure beneath it.


How Many Sessions Do You Need?

For most patients focused on acne scarring, I recommend a series of 3 treatments, spaced 4–6 weeks apart. This is the standard protocol for our standard Microneedling treatment ($400/session), and it gives your skin the time it needs between sessions to fully heal and generate new collagen before we stimulate the process again.

That said, individual needs vary. Patients with deeper or more widespread scarring may benefit from additional sessions beyond the initial three. This is something we assess together as we see how your skin responds.

For patients who want to accelerate and enhance their results, we also offer Microneedling + PRF ($600/session). PRF—Platelet Rich Fibrin—is derived from a small draw of your own blood. The plasma, which is naturally rich in growth factors, is applied directly to the skin during the microneedling treatment. This combination supercharges the healing process: more collagen, more skin cell regeneration, and more noticeable improvement to acne scars and overall skin quality.

Many of my patients who are treating moderate to significant acne scarring choose the PRF upgrade specifically because of how much it amplifies what microneedling alone can do.


What Type of Acne Scars Respond Best?

Not all scars are created equal, and it's worth having an honest conversation about what microneedling can and can't do.

Responds best to microneedling:

  • Rolling scars (broad, wavy depressions)

  • Boxcar scars (defined edges, shallow to medium depth)

  • Post-inflammatory hyperpigmentation (dark spots left after acne)

  • Superficial textural irregularities

May need a combined approach:

  • Ice pick scars (deep, narrow, steep-walled) often require additional treatments—such as chemical peels or laser—for optimal results

  • Very deep or severe scarring may benefit from a combination treatment plan

During your consultation, we'll evaluate your specific scarring pattern and build a realistic plan around your goals. In some cases, pairing microneedling with a chemical peel between sessions can accelerate results—particularly for patients dealing with both texture and hyperpigmentation.


Preparing for Your Microneedling Treatment

Getting the most out of your sessions starts before you walk in the door.

One Week Before

  • Avoid blood-thinning medications and supplements if medically appropriate (ibuprofen, fish oil, vitamin E, turmeric)

  • Stay well hydrated

24–48 Hours Before

  • Avoid alcohol

  • Skip intense sun exposure and tanning

Day Of

  • Arrive with clean skin—no active skincare products or makeup

  • Eat beforehand to keep your blood sugar stable

  • A topical numbing cream is applied prior to treatment for your comfort

After Treatment

  • Avoid direct sunlight for 24 hours and wear SPF daily

  • Skip retinols, acids, and active skincare for a few days post-treatment

  • We'll walk you through a full aftercare protocol at your appointment

For optimal results, we also recommend pairing your microneedling series with medical-grade skincare at home. Products formulated at the right concentrations make a meaningful difference in how your skin heals and maintains results between sessions—and we can point you toward the right options for your skin type.
